The Member hereby purchases a membership in the health fitness club facility operated by Anatomy located at 1212 Lincoln Road St 204, Miami Beach, FL 33139 for a period of time beginning today, and continuing for 12 months thereafter. Membership will continue on a month to month basis until terminated by the member. I understand that monthly dues will be billed on the 25th of each month and shall be billed to the credit card used for payment today ---

Within the contract term, this membership may only be cancelled with proof of relocation of a minimum 25 miles from club or medical note from a doctor and will require one (1) month of membership dues collected at time of cancellation. After the 12 month term, the membership may be cancelled at any time with 30 day written notice by the Member to Anatomy, at which time final Monthly Membership Dues will be deducted from the bank account of the Member or billed to the credit card account of the Member.
