3 Month commitment will auto renew month to month after the 3 months is up. Payment Authorization: I, the undersigned, authorize my financial institution (identified above) to pay monthly drafts on my behalf to FITNESS 4 ALL (the "club") in the amount of 199.99, plus applicable tax, beginning [firstinstallmentdate]. These payments shall continue until I submit written notice of my cancellation to the club. I am obligated to the Training payment terms and unpaid account balance due upon cancellation. I agree you shall be fully protected in honoring these drafts, and that the club's treatment of them and rights in respect to them shall be the same as if they were personally signed by me. I understand I must notify the club of any account changes at least 30 days before the start of the next billing cycle. I understand that I will incur an additional fee of $10 per month whenever there is a payment default on a credit card, or a $30 fee for a payment default on a checking account and authorize the club to charge such fee in addition to my subscription charges set forth above. LENGTH OF TERMS: THIS AGREEMENT is a legal binding obligation for the length of time as stated on the front of this Agreement for which I, the primary member, am financially responsible. I agree that if I fail to pay when payment is due that I may, at the club’s discretion, be required to pay the entire balance due immediately. If collection or legal services are needed to collect any unpaid amounts, I acknowledge that I am responsible for all costs of collection, including attorney’s fees and court costs incurred. All claims, terms and conditions of this Agreement are associated with this club. Fitness 4 All reserves the right to increase subscription fees upon 30-day written notice. RECIPROCITY: I agree and understand I will be charged the then current subscription fee of the Fitness 4 All center I most readily use. Buyer agrees to all policy's and club terms

NOTICE OF CANCELLATION I acknowledge that I may only cancel by signed, written notice to the club at 26495 Southpoint Rd. Perrysburg, OH. 43551 delivered personally or postmarked before midnight of the third business day after which I signed this Agreement. The club will return, within 10 days of receiving my cancellation, any payments made. Upon completion of the initial term set forth above or as extended as provided in this Agreement, the subscription will automatically renew on a month-to-month basis unless I cancel my subscription, in writing, at least 30 days prior to the end of my subscription. I am obligated to pay monthly subscription fees regardless of whether I use the club or its services, activities, equipment or facilities. I must give notice at least 30 days before the start of the next billing cycle to terminate my subscription at the end of the current billing cycle ; otherwise I will incur an additional month’s charge which will automatically be charged to the payment method set forth above. If I elect to cancel my subscription prior to completion of the initial term as set forth above or extended herein I will be charged a cancellation fee of $150 in addition to any subscription charges then due.
